Implementation- 1. Contacts parent/child care provider and initiates treatment according to policies. Conducts ongoing assessment of child during each visit, and modifies plan and short term goals/outcomes accordingly. Submits Amendment Form according to Procedure. Terminates therapy when goals are met, or child no longer meets eligibility criteria for services, and submits documentation for discharge to Coordinator.
